How to get ready to Register:

1. Please click here to open the USYSA Membership Form – (If you have dial up internet, give the form some time to load! See the link at the bottom of this page if you don't have Abode Acrobat Reader) Print out the form and complete it, you should start with Last Name - there are two places the parent or guardian must sign. The medical release part of the form in the lower left must be completed in its entirety. New players to McSwain Soccer must prove their age by providing a copy of Certificate of Live Birth. Uniform size is very important, bring your children to registration to try on the uniforms.

2. Click here to open the Parent Volunteer Form print this form out and complete it. Volunteering is fun and we need your help to make things happen!

3. Supply a small (1" x 1.5") photo of your child for the player pass. Please write your child's name on the back of the picture. (Polaroid pictures can NOT be used). The little rounded sticker ones from the school packages are perfect. You can e-mail a digital photo to Registrar(at)McSwainSoccer.org

4. Make a check out to McSwain Soccer League. The prices this season are: First Child - $60 Second Child and additional children - $55 each. (There will be an additional $10 late fee after May 5th).

5. Bring the completed membership form, copy of Certificate of Live Birth (if new player), volunteer form, picture, your checkbook and your children to:
